I had the same problem with the post-op abductor pillow. I'd always find it on the floor or in the bed the next morning.
Finally, I cut off the Velcro straps on the hospital issued pillow and used a thing called GripTwists to atttach it to my legs before going to sleep.
GripTwists are just some wire inside a foam core, which you can twist together into any shape.
It stays there and it's comfortable.

To make it work, I had to slice a hole in the middle of the abductor pillow to run them through.
